A cross-sectional study to ascertain what the Singapore population would regard as material risk in the anaesthesia consent-taking process and identify demographic factors that predict patient preferences in medical decision-making to tailor a more patient-centered informed consent. A survey was performed involving patients 21 years old and above who attended the pre-operative evaluation clinic over a 1-month period in Singapore General Hospital. Questionnaires were administered to assess patients’ perception of material risks, by trained interviewers. Patients’ demographics were obtained. Mann–Whitney U (...) test and Kruskal-Wallis one-way analysis of variance was used. Statistical significance was taken at p < 0.05. Four hundred fourteen patients were eligible of which 26 refused to participate and 24 were excluded due to language barrier. 364 patients were recruited. A higher level of education, being employed and younger age group are factors identified in patients who wanted greater participation in medical decisions. Gender, marital status, type of surgery, and previous surgical history did not affect their level of participation. The complications most patients knew about were Nausea, Drowsiness and Surgical Wound Pain. Patients ranked Heart Attack, Death and Stroke as the most significant risks that they wanted to be informed about in greater detail. Most patients wanted to make a joint decision with the anaesthetist, instead of letting the doctor decide or deciding for themselves. Discussion with the anaesthetist is the preferred medium of communication compared to reading a pamphlet or watching a video. Age and educational level can influence medical decision-making. Despite the digital age, most patients still prefer a clinic consult instead of audio-visual multimedia for pre-operative anaesthetic counselling. The local population appears to place greater importance on rare but serious complications compared to common complications. This illustrates the need to contextualize information provided during informed consent to strengthen the doctor-patient relationship. (shrink)

Mackie attempts to revise Locke's theory of the identity of persons. Locke's equation of personal identity with the unity of consciousness is set out. Mackie examines various objections to Locke's theory and his use of puzzle cases is highlighted. Recent theories of personal identity, such as Parfit's, are critically discussed. Mackie suggests that the equation of personal identity with the continuity of neurophysiological structure would resolve some of the difficulties associated with an account of personal identity.

Stephen Finlay’s proposed methodology for defending the central theses of his impressive Confusion of Tongues is an underexplored aspect of this work.1 1 His official methodology is analytic : A reduction of normative to non-normative vocabulary. Here, I argue that taking this official line at face-value forces the reader to conclude that the reductions at the heart of that book cannot be correct. In contrast, a philosophical methodology that does not proceed via analyses would better support those reductions, then understood (...) as non-analytic. As we’ll see below, there are independent, empirical reasons to reject a philosophical methodology that proceeds via analyses. I conclude that there is a method for the study of ethical language that better serves Finlay’s primary philosophical aims. The lesson, though, is generalizable: Given the array of normative and meta-normative theories, we want a semantics for normative discourse to be capable of expressing, we should expect that the more plausible such views will not be defended via analyses. This paper addresses two related questions. First, what is involved in giving a distinctively realist and naturalist construal of an area of discourse, that is, in so much as stating a distinctively realist and naturalist position about, for example, content or value? I defend a condition that guarantees the realism and naturalism of any position satisfying it, at least in the case of positions on content, but perhaps in other cases as well. Second, what sorts of considerations render a distinctively (...) realist and naturalist position more plausible than its irrealist and non-naturalist rivals? The answer here focuses again on theories of content and is wholly negative. I argue that the standard array of arguments offered in support of realist and naturalist theories in fact provide equal support for a host of irrealist and non-naturalist ones. Taken together, these considerations reveal an important gap in the recent philosophical literature on content. The challenge to proponents of putatively realist and naturalist theories is to insure that those theories so much as state distinctively realist and naturalist positions and then to identify arguments that support what is distinctively realist and naturalist about them. Wykstra’s paper defends two objections to my reasoning in The Wisdom to Doubt. One says that we in fact do take evidence to be representative of all the relevant evidence that exists when forming the judgment that it makes some proposition probable, the other that our judgments as to the representativeness of evidence are often justified, and can be justified even in matters of religion. Both objections are instructive but ultimately unsuccessful, as I show here.
